What is Project ECHO?
Project ECHO is an all-teach-all-learn telementoring model focused on building the capacity of health providers. Seeing clear alignment with our mission and vision, AIDPH was credentialed in the ECHO model in 2020 to establish a rural oral health ECHO hub. Throughout 2021, AIDPH approached stakeholders in rural communities to design programming that meets their needs.
Our Rural Oral Health ECHO follows The Three Domains Framework using the lens of rurality and health equity. Each virtual session includes three key elements:
- Expert didactic presentation
- Participant Q&A
- Challenge for Think Tank
Participants take this knowledge back to their communities to implement in their daily work, building the capacity of our oral health professional workforce.

We are proud to offer these sessions in collaboration with University of Nevada Las Vegas, CareQuest Institute for Oral Health, and the OPEN Rural Network Response Team who will join AIDPH in forming our ECHO Hub. Experts change with each session based on didactic presentation topics.
Each monthly session features a strategic conversation on a rural oral health challenge that a provider or site is currently facing. Challenges can be about a clinical concern, program framework, outreach effort, funding need, or issues related to the social determinants of health in rural areas. Do you want expert feedback from community thought leaders and a written report with customized strategic recommendations? We'd love to have you share at our session!
Earn a Certificate in Special Topics in Rural Oral Health (2022)
AIDPH has developed a certificate program for ECHO program participants to pursue as a formalized pathway signaling expertise and training in rural oral health. The certificate program is self-guided. After each monthly ECHO session, certificate candidates can choose to take a competency assessment. Assessments are 5 questions and must be completed with 100% accuracy to be eligible for certificate consideration. You may retake each quiz until you achieve that score. Upon completion of eight competency evaluations, we will send you a self-reflection evaluation for you to complete prior to awarding your certificate. The special topics in rural oral health certificate pathway for 2022 is completely optional - participants can attend ECHO sessions without choosing to complete competency assessments.
You can participate in each session live (preferred) or on-demand using the recordings below. Each session includes a link to complete a brief competency evaluation. Email programs@aidph.org to validate your certificate after attending eight sessions.
AIDPH looks forward to offering a new certification focus area for 2023. Details will be posted as soon as they become available.
Title: Population Health Management in Rural Oral Health

Abstract: Population health management is a health care delivery approach involving patient stratification by clinical indicators to provide more personalized and integrated care. Subpopulations of patients can receive more specialized and personalized care based on clinical needs and outcomes thereby streamlining care delivery and payment models. To achieve effective population health management, data analysis across care team specialties should be used in concert to identify trends, anticipate needs, and implement responsive care on a population level. In rural oral health care settings, a regionalized approach can be helpful to advancing community-driven solutions that support population health management. In this session financial models, health messaging, and data trends will be presented for application in rural oral health.

Abstract: This didactic portion of session will introduce the concept of risk-stratified care for dental teams, with a focus on population health management for rural populations. Participants will explore the multi-step process of risk stratification, discuss a pertinent case example, and learn about both the health and economic benefits of this strategy. The presenter will also review environmental opportunities and challenges for practicing risk-stratified care, to set the foundation for the case presentation discussion.

Title: Teleprevention & Teledentistry in Rural Oral Health

Nathan Suter, DDS is a teledentistry enthusiast, practice owner and Chief Innovation Officer at Enable Dental and practice owner of Green Leaf Dental Care. He is passionate about solving problems that can help create access to dental care. He began his career in public health developing programs that serve underserved communities. That experience has allowed him to see the gaps in our dental care delivery system. He is an advocate in disrupting the system by leveraging technology and bringing ideas that are outside the box and innovative. He was awarded 2019 Missouri Dentist of the year by the Missouri Dental Association and awarded ADA's 2020 Top 10 Dentist Under 10 Years in practice.
Abstract: Teledentistry and telehealth can be challenging in a rural environment. Rural populations often face significant challenges in accessing optimal oral health care, including insufficient broadband infrastructure and lack of rural care clinics. Teleprevention models when used in tandem with other asset-based rural care delivery models can extend opportunities to access dental care and improve the patient health experience. In this session, the presenter will share innovative teleprevention models for whole-person health care management, including oral health in rural settings. Participants will be invited to consider non-traditional approaches to implementing teleprevention and teledentistry in the rural oral health care environment. The presenter will explore the emerging dental delivery model of teledentistry. Dr. Suter will cover a variety of domains, such as definitions, terminology and legal and regulatory standards. In summary, this presentation will show how through the utilization of care coordination, and teledentistry methodology we have spearheaded an innovative approach to increase access to care to vulnerable populations.

Non-Invasive Caries Management in Rural America: Brush-on Treatments and Behaviors to Change

Jeremy Horst Keeper DDS, PhD is the director of clinical innovation at CareQuest Innovation Partners, an organization whose mission is to improve oral health for all by advancing innovation to transform access and outcomes for underserved populations across the U.S. As a practicing pediatric dentist, biochemist, and educator, Jeremy is known for investigating the strengths and limitations of Silver Fluoride therapy, helping to develop SMART fillings, and accelerating the adoption of maximally effective, minimally invasive caries management techniques. At CareQuest, Jeremy is driving the development of better treatments, preventives, and diagnostics to improve access to person-centered care, integrate oral health into overall health, and stop dental caries.
Abstract: We now have a portfolio of treatments for dental caries that do not require needles or drills. The requirements for caries management have changed; only indirectly accessible cavitated caries lesions still require traditional approaches (cavitated class IIs). Caries lesions and the disease of caries itself are now be treated by hygienists, assistants, dental therapists, community health workers, and medical teams. Medicines for treating caries will be introduced, including SDF, Glass ionomer, and Curodont. This pharmacopeia will be discussed from the perspective of rural access amplified by provider-to-provider teledentistry, mobile/portable models, and empowering a more broad oral health workforce.

Title: Honoring the Lived Experience of LGBTQ+ People in FQHCs

Abstract: Implementing a whole-person health perspective is integral to providing effective oral health care to LGBTQ+ people. The lived experience of historically excluded, marginalized, and minoritized communities often impacts their ability to engage in self-care, including their oral health. FQHCs play a major role in facilitating accessible and affordable health care to underserved communities, including LGBTQ+ people, and can take steps to maintaining inclusive spaces. During this session, participants will learn about best practices and promising approaches to oral health care from the perspective of Chase Brexton Health Care, a community health center providing integrated health care to LGBTQ+ people in the Baltimore area. An overview of The Center for LGBTQ Health Equity, a clinic dedicated to health care services, education, training, research, and advocacy that end health disparities within the LGBTQ+ community, will be provided. Tools and resources for oral health professionals will be shared.

Title: Predictive and Personalized Care in Dental Care

Dr. Sean Boynes is a dentist anesthesiologist who currently serves as President of Dental Medicine Consulting and the Science and Research Lead at the American Institute of Dental Public Health. Dr. Boynes is a recognized oral health leader, he has held executive positions and professional appointments in academia, public health, insurance, research, and care delivery. During his 20-year career, he has consulted with hundreds of people working in health centers, rural health clinics and private dental practice, as well as technology start-ups, community organizations, and some of the largest businesses and universities in healthcare. A former community health center dental director and National Health Service Corps Alumnus, he received his B.S. from Lipscomb University and D.M.D. from the University of Pittsburgh. He completed his anesthesiology residency at the University of Pittsburgh Medical Center. Dr. Boynes is a unique thought leader with over 125 professional publications and receiving honors from such organizations as the South Carolina Association of School Nurses, the National Network for Oral Health Access, Pennsylvania Dental Hygiene Association, National Health Service Corps, and America’s Toothfairy.
Abstract: With the paradigm changes occurring in oral health, the current workforce is feeling the stress and pressure from the last couple of years. Many medical and dental care teams feel they do not have enough time to move beyond problem-based care encounters and re-connect with the joy of practice. An approach moving forward to help alleviate these issues is personalized care management. The process incorporates population health and data driven insights within a personalized care approach to help patients achieve the best health and quality of life possible. The main drivers of this methodology is to prevent chronic disease, stabilize current chronic conditions, and prevent acceleration to higher-risk categories and higher associated costs. Participants will learn the cornerstones of a personalized care structure and discuss the opportunities of a personalized management structure.

Title: Whole Health & Wellbeing: The intersection of mental health, substance use disorder, and oral health

Shauna Reitmeier serves as the Chief Executive Officer at Alluma, formerly Northwestern Mental Health Center. Shauna has led the organization through dynamic growth and development, including the successful implementation of the Certified Community Behavioral Health Clinic model of service delivery. Under Shauna's leadership, Alluma doubled its revenue, increased its staff by 35 percent, and doubled the number of clients served.
In addition to her leadership and management responsibilities at Alluma, she is a Licensed Graduate Social Worker and sees clients at the Crookston clinic location. Shauna holds a Master of Social Work degree from the University of Michigan, Ann Arbor, and a bachelor’s degree in social work from the University of North Dakota, Grand Forks.
Before joining Alluma, Shauna worked with the National Council for Behavioral Health, providing contract management and technical oversight to integrate primary and behavioral healthcare across the country. While serving in multiple roles at Washtenaw Community Health Organization, she gained extensive experience in quality and process improvement, strategic planning, and integration of systems.
Shauna currently serves on several boards, including the National Council for Mental Wellbeing, National Association of Rural Mental Health, The MN Association of Community Mental Health Programs, Convergence Integrated Care, Northwest Council of Collaboratives and active in the Crookston Lions. Most recently, Shauna was appointed by MN Governor Tim Walz to serve on the newly created Blue Ribbon Commission on Health and Human Services.
Abstract: Many adolescents and adults with a mental health (MH) or substance use disorder (SUD) have oral health issues stemming from myriad factors. Shauna Reitmeier, CEO at Alluma, will share the impact MH and SUDs have on oral health. This presentation will also highlight how the integration of oral health providers with MH and SUD organizations can facilitate access to care through cross-system multi-disciplinary screening for improved outcomes and overall wellbeing.

Title: Your Role in Advocacy and Policy Change

Chloe Van Zandt serves as Virginia Health Catalyst's Director of Water. In this role, she leads Catalyst's water equity initiatives to ensure everyone in the Commonwealth has equitable access to safe, trusted, fluoridated drinking water that is preferred. Previous to this role, Chloe led Catalyst's grassroots advocacy efforts to successfully include an adult dental benefit in Medicaid and increase Medicaid dental reimbursement rates. Before joining Virginia Health Catalyst, Chloe worked in higher education where she coordinated community organizing to engage students in activism. Chloe earned her Bachelor of Science in Communication from Boise State University and her Masters of Public Health from George Washington University.
Abstract: As public health professionals, you most likely see a need for system-level change, which addresses the root cause of a problem, to improve the health of the populations writ large. Policy change is the most effective tool to achieve system-level change, but you don't have to be a lobbyist or policy-wonk to make a difference. By building relationships and creating a community, you can help advocate for policies that will benefit your community. You can have a lasting impact on the health of generations to come. This session will discuss how public health professionals can effect change by developing a personal and customized approach to advocacy work. Attendees will learn from the process of developing a community engagement model. Chloe Van Zandt will share her experience and lessons learned while leading the grassroots movement to include an adult dental benefit in Virginia's Medicaid program

Title: Supporting the Oral Health and Wellbeing of Rural Veterans

Abstract: More than 85% veterans do not qualify for dental care through the VA due to strict congressionally-mandated eligibility. As a result, access points to dental care can be fragmented for many veterans, but particularly for rural veterans who experience geographic barriers to care. In this ECHO session, AIDPH and CareQuest Institute will share a newly-launched visual report evaluating the oral health of veterans living in rural areas. Results indicate that rural veterans are experiencing more negative oral health outcomes and chronic disease conditions compared to urban veterans. This report is released in conjunction with an expansion to our veteran oral health data dashboard featuring interactive visualizations of rural data.
