Background and Purpose
The American Institute of Dental Public Health (AIDPH) and the National Network for Oral Health Access (NNOHA) are partnering to support the collection and dissemination of innovative oral health care delivery for veterans.
The purpose of this survey is to collect information regarding emerging, promising, and best practice programs related to veteran oral health in community health centers around the country. Survey questions will assess program background and history, strategies to improve veteran oral health, cost of delivery of oral health care for veterans, as well as approaches to enhance the care experience of veterans. The results from this survey will be reviewed by a workgroup of experts identified by NNOHA and AIDPH to create a resource guide. This project is funded through NNOHA's HRSA National Training and Technical Assistance Partnership.
Survey Instructions
Who should fill it out? Individuals in leadership positions within their organizations. This includes, but is not limited to, a dental director / chief dental officer, medical director / chief medical officer, office manger, or outreach coordinator.
Please create only one submission per organization.
You should answer each question to the best of your abilities. If you are unable to answer a question, please state ‘unknown’, N/A, or leave blank. We expect submissions to take approximately 15 minutes to complete. Submissions will be entered into a raffle for a $50 Amazon gift card.

What You Need to Submit
Most of the questions are multiple choice or narrative/describing your programmatic approach. We are also requesting links to articles, evaluations, or other data that support outcomes from your program. We also encourage you to submit any information, like recruitment materials or educational resources, that could be shared as a helpful tool with others. If you have many resources to share, you may want to include them all in a Google Folder and link them in the survey.
Data Handling and Record Keeping
The facilitators are responsible for ensuring the accuracy, completeness, and timeliness of the data reported. All source documents will be stored in a password protected drive.
